The story of a 60-year-old woman in my neighborhood

I used to see her occasionally whenever I passed by, but since I hadn't seen her for a few months, I asked how she was doing... and I heard she was hospitalized with cancer (she is currently in a nursing hospital).
She had surgery, and it was that expensive robotic surgery; her insurance consists of two types: 5th generation actual cost medical insurance and integrated nursing/caregiver expense insurance.
Based on the few times I spoke with her, and what another neighbor who knows her well said—that she lived life passionately—I thought, "Ah, she must have worked hard as a dual-income household!!!"
As it turns out, she has been a full-time housewife, and her family's financial status isn't even middle class.
Regarding the 5th generation medical insurance, she didn't switch from maintaining 1st or 2nd generation insurance; she simply signed up for it very late.
The 3rd generation medical insurance was released in 2017 and sold until mid-2021; I wonder what she was doing during that period that she ended up with 5th generation insurance, where the out-of-pocket expenses are so high....
Besides the actual cost medical insurance, is the integrated nursing/caregiver expense insurance the only other insurance she has..
To be honest, my inner thought was that it was pathetic.
If she had worked even a part-time job for 3–4 hours a day consistently since her 40s, she could have maintained at least cancer insurance that provides a diagnosis fee and caregiver insurance...
It's not like she gave birth late, either.
Even if she had given birth late, part-time work would have been perfectly possible.
In the end, it seems her husband is working hard, even cutting down significantly on his days off.
