If you look at it positively, does it mean they are pure or unpretentious?
Of course, some people are like that—either they choose not to dress up, or they simply don't know how.
However, it could also be someone with strong stubbornness, strong personal convictions, or someone with a very intense private world. Someone with a strong ego who thinks, "I am who I am..."
If they fall into the former category, they are likely someone who lacks worldly experience or has much to learn.
If they fall into the latter, they are someone with very strong personal thoughts and perspectives.
If you offer clumsy advice or criticism, the first type might worry privately, thinking, "Am I really in the wrong?" But the second type is highly likely to immediately view you as an enemy or interpret you as an uncomfortable, alien presence.
If they are the type with a strong personal world or strong personal beliefs, they are close to being a true believer, a person of absolute conviction, or a person of unshakeable conscience, making it highly likely that conversation with them will be difficult. Negotiating or compromising with such a person is nearly impossible. It is at a level of impossibility.
The fact that someone does not or cannot dress themselves up could mean:
1. They are someone who is less worldly, and thus a pure person.
2. They are someone with extremely strong personal convictions, stubbornness, or thoughts (to the level of being extremely "my way or the highway").
To know exactly which type they are, one would have to observe their words, actions, and surrounding environment.
