The real, ultimate reason why you actually fail

To be honest, there is a specific reason why you fail. In fact, I am 100% certain of this right now.
But why does it happen? Why, even when things were rigged, do people ask, "Isn't that person at fault?"

Let's follow a conversation labeled 'A'. If they didn't know, they wouldn't realize the conversation was staged. But then, they catch on.
They strike once. They say, "There's no need to do that, so why are you manufacturing this conversation?"
But it still feels strange. So they strike again. "Why are you continuing this conversation and steering it toward a result just to say, 'This person did this'?"
And suddenly, the response is, "I'm sorry. I won't do that again."
